Dog vaccination schedule record

Follow your dog's vet visits in order: the date, what the vet gave, and the next visit date they set. Your vet decides the schedule. This sheet keeps the sequence easy to read.

How this differs from a general vaccination record

The general vaccination record lists vaccines one by one. This sheet follows visits in order, which suits a dog who is seen often in the first months and then less often. Use whichever reads more clearly to you, or keep both.

Questions to bring to the first visit

  • What do you recommend for my dog, given their age, home, and activities?
  • When should the next visit be, and what will it cover?
  • What should I watch for after a visit, and who do I call if I am unsure?
  • What paperwork will you give me for daycare, boarding, or grooming?
  • Can I get a copy of the record after each visit?

Filling in the sheet

  • Write the visit date and copy what the vet gave exactly as it is listed.
  • Enter the next visit date only if your vet gave you one.
  • Add a note for anything the vet asked you to do before the next visit.
  • Keep the clinic's own paperwork. This sheet is a summary, not a substitute.

If your dog was adopted or rescued

Adoption paperwork often lists vaccines given before the dog went home. Copy those onto Visit 1 and write given before adoption in the notes. If the paperwork is missing or unclear, ask the shelter or rescue for a copy, and ask your vet how to proceed. Do not fill gaps with guesses.

What this is not

The sheet does not know which vaccines a dog needs or when. Your veterinarian decides that, and the dates in this sheet are only the ones you typed in.

Common questions

What vaccines does my dog need and when?
That is for your veterinarian to decide based on your dog and where you live. This sheet does not recommend vaccines or dates.
Can I use this for a puppy and keep using it as an adult?
Yes. Add rows as visits happen. The sheet keeps adding to the same record in this browser if you save it.
How is my dog's information stored?
Only in your browser, and only after you press Save in this browser. Nothing is uploaded to WhiskerMatch.
