Pet emergency info card

Put the facts someone would need if you could not speak for your pet on one small card: who the pet is, how to reach you, and the vet to call. Print it and keep copies where they will be found.

What belongs on the card

  • The pet's name, species, and a short description.
  • A microchip number and the name of the registry that holds it, if your pet is chipped.
  • Two people to call, in case the first does not answer.
  • Your vet and an emergency clinic, with phone numbers.
  • Medical needs or allergies, written as your vet told you.

Where to keep copies

  • In your wallet or phone case.
  • On the fridge or by the front door, where a sitter or neighbor can see it.
  • In the carrier, the car, and any go-bag you keep.
  • With your sitter, boarding place, or anyone who has a key.

Check your microchip registration

A microchip number helps only if the registry has your current phone number and email. If you have moved or changed numbers, update the registry. The card is a good prompt to check.

Keep it current

Update the card when a phone number, clinic, or medical note changes, and replace the old copies. A card with a wrong number is worse than none.

Common questions

Should I put my home address on the card?
That is your decision. Many people leave the address off and rely on a phone number. If you include it, think about who will see the card.
Does this replace a collar tag or microchip?
No. It is an extra copy of key facts for people who help you, such as a sitter or a neighbor. A tag and a registered microchip are separate.
Is the card stored online?
No. It is made in your browser. If you press Save in this browser, a copy is kept on this device only.
