What to write down for each medication

  • The name exactly as printed on the label or prescription.
  • The dose and how often, copied from the label or your vet's written instructions.
  • The start date, and the last day if there is one.
  • A refill date, so a refill is not a surprise on a weekend.
  • Who prescribed it, and the clinic's phone number.
  • Anything the label or clinic told you about giving it, such as with food.

Copy, do not calculate

This tracker has no dose calculator and no suggestions. Every dose in the table is something you typed from a label or from your vet. If the instructions change, add the new instruction and who gave it, rather than writing over the old one.

Using the printed week sheet

The printed table adds seven blank day boxes after each medication. Tick or initial a box when a dose is given. If more than one person gives medication, have each person initial the box so nobody gives a dose twice.

When something does not look right

A missed dose, a question about the label, or a change in how your pet seems is a question for your veterinarian or the clinic's emergency line. This page cannot answer it. Keep the clinic and emergency numbers at the top of the sheet.

Bring it to the vet

A printed medication list, including supplements and anything given occasionally, saves time at appointments and helps a new clinic or a sitter see the full picture.

Common questions

Does this suggest a dose or tell me when to give a medication?
No. It holds only what you type from the label or your vet's instructions. Dosing is a decision for your veterinarian.
Will it remind me to give a dose?
No. It can export the refill dates you enter to a calendar file, and you choose any alert in your own calendar app. It does not send reminders and has no account.
Is my medication list uploaded anywhere?
No. It stays in your browser, and only if you press Save in this browser. You can remove it at any time with Clear saved copy.

Free to use, nothing uploaded
