A personal record, not medical or legal advice. ## What a useful vaccination record includes A record is only as good as the paperwork it was copied from. The point of this table is to put the facts from your vet's papers in one place you can find quickly. - The name of each vaccine exactly as it appears on the certificate or invoice. - The date it was given, not the date you entered it. - The clinic or veterinarian who gave it. - The next date your vet gave you, if they gave one. - The lot or batch number if the paperwork lists it. - A note about anything the clinic told you to watch for or report back. ## How to fill it in - Start with the papers you already have: certificates, invoices, adoption paperwork, or the clinic's patient portal. - Copy names and dates as written. Do not translate or abbreviate them. - If a date is missing, leave it blank or write unknown. A guessed date can cause more trouble than a gap. - Add a row after every visit while the paperwork is still on the table. ## Who may ask to see it Boarding places, daycares, groomers, training classes, and new veterinary clinics often ask for vaccination records. Each sets its own rules, so ask what they accept. Some want the clinic's own certificate and will not take a home-made table, which is why this sheet works best as your working copy beside the official paperwork. ## Keeping it current - Use Save in this browser so the table is there next time on the same device. - Print a copy for the carrier bag, the car, or a folder for the sitter. - Export the next-due dates you entered to your calendar and choose your own alert. - Ask your vet which dates to trust. If a boarding place or daycare needs proof, ask them what they accept and request the clinic's own paperwork. - **Where is my information stored?** Only in your browser, and only if you press Save in this browser.
